Survey to Assess Number of Unvaccinated Islanders Who Are 65 and Older

To better understand the current need for vaccinations from the San Juan County 65+ community, San Juan County Health and Community Services asks that those who meet ALL of the four measures below fill out a simple form.

PLEASE BE:

– A San Juan County resident (short term or long term), and

– Age 65 or older, and

– Currently UNVACCINATED, and

– Interested in being vaccinated.
